If you do not have permission to use Dave Nestler's images please find a different tube!
Open a new canvas 550 x 400 with a white background
Choose your shape tool set to rectangle with the stroke colour transparent and the center black
Draw four rectangles on the screen. For my tag I used the distribution below but you may want to experiment

Convert this to a Raster Layer
Now from the main menu choose
Effects>Distortion Effects>Ripple
and set it up as follows:-
Horizontal - 50.00
Vertical - 50%
Amplitude - 22
Wavelength - 40
Edge Mode set to Transparent
Main Menu>Layers>Duplicate
Main Menu>Image>Mirror
Main Menu>Image>FLip
Main Menu>Layers>Merge>Merge down
Now back to the ripple effect with the same settings
Main Menu>Layers>Duplicate
Main Menu>Image>Flip
Now got back to your shape tool reverse the stroke and fill colours and make the line width 2
Draw a rectangle around your image just away from the edge
Now add your tube as a new layer
If needs be use your magic wand and the border to remove any over-hanging tube
Finally choose your text tool and using a blocky font with the fill colour set to white place the name ont he tag
Main Menu>Effects>3D Effects>Drop Shadow using the following settings
Vertical: -1
Horizontal: 2
Opacity: 100
Blur: 2
Click OK and go straight back to the drop shadow effect
Set the Vertical and Horizontal to each others value so:
Vertical: 2
Horizontal: -1
Lower the Opacity of this layer to 75%
Now get a more script like font and add the two smaller names to the underneath of the main text
Add a drop shadow that looks good to your smaller text and the same one to your tube
Now save the PSPimage and export as a JPEG
