You will need nothing more than Paint Shop Pro X and its default filters to complete this tutorial.

Make a new canvas 560x210 with a white background
Grab your shape tool and draw a square with a width of 2, colour #000000
Convert this to a raster layer
Right click on the layer and duplicate it three times
Now using your move tool spread them evenly across the canvas just under the top of the canvas
Make all but the first box invisible
Select your magic wand tool
Copy your tube and paste as new layer on your main canvas
Place your tube so that you can see the part you wish showing inside the first box
Select the box layer
Click with your magic wand inside the box
From the Main Menu choose Selections>Modify>Expand by 1
Then choose Selections>Invert
Reselect your tube layer and hit delete
Make the background layer invisible then right click on the layers palette and choose Merge>Merge visible
Make this new layer invisible
now make the next box visible and repeat the above for all the other boxes
Once that is don grab your shape tool again and draw a square evenly around the outside of each box
Convert to raster
**Hint** If you draw one then duplicate that layer then you can position each box indiviually around each existing box
Now grab your selection tool set to rectangle
Select the top left corner of the first box (without the tube inside)
**Note** Try and keep the selection at the same edges as the main box it surrounds
hit delete and repeat for all the corners on each outside box

Main Menu>Effects>3D Effects>Drop Shadow
Use the following settings
Vertical : 0
Horizontal : 0
Opacity : 100
Blur : 11
The colour should come from your tube
**Note** It should be very difficult to see this drop shadow
Next get your pen tool set to the same colour as the box lines
Draw one line between the middle 2 boxes

Objects Align>vertical center in canvas
Convert to raster
Now draw two lines between the outer two boxes on both sides. Roughly at divisions of 3

Convert to raster
Now grab your shape tool set to rectangle
Stroke : Black
Fill : white
Same width as before
Draw a box under the main 4 boxes
Convert this to a Raster Layer
Inside this place the name for the tag using whatever font you wish
I used Tangerine XCn size 48
Add to this the same drop shadow as you did the outer lines round the boxes
Convert to raster
Remove the fill from the colours(make the fill transparent)
Now draw a large box that comes out of the center of the first box and finishes in the center of the 4th
This must also pass through the center of the name box
Convert to raster
Grab your selection tool set to rectangle and select the parts of the last box added that we do not wish to see
It should start at the outer line round the box, disappear under the name box, reappear on the other side and then finish on the outer line round the fourth box
Now make all layers visible
Add your copyright information if needed
Next Save your file as a PSPimage and Export if necessary
